Preschool, Kindergarten, Grades 5 & 9 students must have current physicals on file by August 24.

All Health records are required at registration.
Students need indicated health forms completed if transferring from:	an Illinois school	out-of-state	out-of-country
Preschool Students entering preschool require a physical.	- Record of immunizations   - Physical exam completed within one year of entering District 202 schools. Must be completed on an Illinois Child Exam Form or comparable form by a physician.	- Record of immunizations   Physical exam completed within one year of entering District 202 schools. Must be completed on an Illinois Child Exam Form or comparable form by a physician.	- Record of immunizations   - Physical exam completed within one year of entering District 202 schools. Must be completed on an Illinois Child Exam Form or comparable form by a physician. Tuberculosis screening is required and documentation of being free from the disease.

K - Grade 4 Students entering kindergarten require a physical.	- A copy of child's kindergarten physical for children in kindergarten through 4th grade and up-to-date immunizations.	- Record of immunizations   - Physical exam completed within one year of entering District 202 schools. Must be completed on an Illinois Child Exam Form or comparable form by a physician.	- Record of immunizations   - Physical exam completed within one year of entering District 202 schools. Must be completed on an Illinois Child Exam Form or comparable form by a physician. Tuberculosis screening is required and documentation of being free from the disease.
