When registering as a new student to Plainfield Community Consolidated School District 202, the following information will be required.
  1. Illinois State Board of Education Transfer paper, if you are currently enrolled in another Illinois school district.
  2. Birth Certificate.
  3. Record of Immunization.
  4. Health Record Requirement.
  5. Proof of Residency, which includes one or two documents from Category A and one or two documents from Category B for a total of three documents:

Category A - One or two of the following:

  • Most recent real estate tax bill, OR
  • Lease agreement for currently occupied residence, OR
  • Closing / settlement statement from the purchase of your home (which includes the address as well as buyers' and sellers' signatures), OR
  • A notarized Student Residency Questionnaire from the owner of a residence within the District boundaries stating student and legal guardian are residing with said owner on a full-time basis, along with the homeowner's proof of residency, OR
  • Prepaid hotel receipt, OR
  • Signed contract to build along with a deposit of $350.00 per student, OR
  • Signed lease to begin renting within the 30 day grace period along with a deposit of $350.00 per student; AND

Category B - One or two of the following:

  • Driver's license
  • Utility bill naming you the responsible person at the address
  • Homeowner/renter insurance for that address
  • Voter card
  • Automobile registration
