Field Trip Checklist

 

 

 

¨      Approval of field trip by Department Chair.

¨      Field Trip Approval form completed (ONE field trip per form).

¨      Sub Request form completed for each staff member on all trips.

¨      Transportation Request form completed.

¨      Field Trip Approval form, Sub Request form, and Transportation Request form completed and submitted together to the Assistant Principal at least four (4) weeks in advance of the trip.

¨      Field trip approved by Assistant Principal.

¨      Overnight and Out-of-State field trips must have prior BOE approval.

¨      Trip Consent form (permission slip) distributed to students.

¨      Signed Trip Consent forms (permission slips) collected from students.

¨      All monies collected for field trips must be deposited/kept with the bookkeepers – DO NOT KEEP MONEY YOURSELVES!

¨      List of students going on the trip given to all staff one (1) week prior to the trip.

¨      Updated list of students going on the trip provided to staff the day before the trip.

¨      Check on transportation the day prior to the trip (recommended).

¨      Accurate attendance given to Attendance Office prior to departure.

¨      Copies of signed permission slips left with principal’s secretary prior to departure.

¨      Multiple-bus trips require attendance listed by bus.
