All kindergarten students entering District 202 are required to have a kindergarten physical exam on an Illinois Child Health Exam form, dated after August 21, 2007, listing all dates of required immunizations.  This form must be brought to your child’s school before classes begin on August 21, 2008. This is a District 202 and Illinois Department of Public Health requirement.  Children with missing health requirements will not be allowed to start school on August 21, 2008. 

Effective January 1, 2008.  A new state law SB641, provides that a vision examination must be included as a part of the health examination required within one year prior to entering kindergarten or the 1st grade, whichever grade the child first enters. Proof of the required eye examination by a licensed optometrist must be submitted by Oct. 15 of each school year. Additional vision examinations at various grade levels may be required when deemed necessary by school authorities

As of July 1, 2005, a new state law went into effect.  All children in Kindergarten will be required to have a dental health exam performed by a licensed dentist.  This form is to be submitted to your child’s nurse before May 15, 2009.
