Health Requirements

Mandatory Dental Exam

As of July 1, 2005, a new State of Illinois law went into effect requiring that all students in Kindergarten, second and sixth grades have a dental exam performed by a licensed dentist. For this school year (2008-09), the exam must be performed sometime after November 15, 2007, and before May 15, 2009.

Following your child's exam, please have your dentist fill out the form required by the State and return it to the Health Office of his/her school no later than May 15, 2009. Exam forms are available by downloading Dental Examination Record and if needed a waiver can be submitted by using the Dental Exam Waiver.

In addition, if you need a referral to find a dentist or financial assistance to obtain your child's exam, please contact your school's Health Office or the Will County Community Health Center Dental Clinic at (815) 774-7300.

Physical Examinations

Illinois State Law (http://www.isbe.state.il.us/research/pdfs/px_requirements.pdf) requires students entering Preschool, Kindergarten, 6th grade, 9th grade, and/or when transferring from an out-of-state school to have their required physical exams and immunizations completed by the first day of school, or they will not be allowed to enter class. Appointment dates will not be accepted. The health history portion of the physical examination form must be completed and signed by the parent in order for the physical to be valid.

Physical examinations may be acquired through your primary medical provider. If you do not have a primary medical provider, you may contact your school nurse or the Will County Health Department at (815) 727-8670.
